Our Community Health Program connects eligible residents with care and support outside of the hospital setting. Instead of creating a new medical system, the community health model offers an innovative, proven way to provide high-quality primary care and preventive services by using a currently available—and often underutilized—healthcare resource: paramedics.

This program helps reduce unnecessary emergency room visits, improves access to care, and supports residents who may have barriers to getting the services they need.
